Diagnosing low back pain with radicular symptoms Most cases of low back back pain comes with radiculopathy. This is where pain in the lower back radiates into the thigh often times. Normally, the result is from compression of a nerve root that usually is found to be involving L4-L5 spinal nerve roots. This compression is most of the time caused by a herniated disk in the stipulated area. Using the straight leg raise test is a sensitive test that helps confirms compression of nerve root in that case of low back pain with radicular symptoms in the legs. The main reason being that, whenever the leg is raised in such manner, movement that occurs in the spine, causes further compression in the area of disk herniation and so causes more pain to transmit on the path of the sciatic nerve, resulting in the feeling of pain symptom similarly in one of the legs which is raised up in an angle, usually between 0-70 degrees from the treatment table. After reaching this angle when therapist adds maximal ankle dorsiflexion, pain increases all the more.
